Requisite Forms for Participation

Unfortunately, we will require some of the forms to be completed manually. (Still a state law.) The forms below will be required, following confirmed registration, to be completed separately. You will get a link to them in your registration confirmation email or may download them here. Please complete and return all forms to the address below NO LATER THAN August 1, 2024 to avoid a late fee of $35.00.  Students will not be able to attend classes until ALL required forms are submitted.

  • [DHMH 896] Immunization Certificate
  • Guide to Regulated Child Care (for your records only)
  • Regulated Child Care Acknowledgement Form

  • [1216A] Asthma Action Plan and Medication Administration Authorization Form
  • [1216B] Allergy and Anaphylaxis Medication Administration Authorization Form
  • [1216C] Seizure/Convulsion/Epilepsy Disorder Medication Administration Authorization Form

Although not required, we request a copy of any IFSP/IEP your child may have in place to help us better serve your child.

Refund Policy

The registration fee of $65.00 ($75.00 after August 1, 2024) and supply fee of $45.00 are non-refundable and non-transferable.

Physical supply donations are not returnable.

Tuition may be refunded only in the case of withdrawal prior to  September 1, 2024, AND once the vacancy has been filed. There are no refunds after September 1, 2024 for any reason and parents are responsible for the full year's tuition regardless of any withdrawal after September 1, 2024.
