COVID-19 Waiver
Until further notice, the COVID-19 Waiver must be signed by any LTRC volunteer, coach, program administrator (Paid or Volunteer), contractual employee, vendor, or other person required by the LTRC officers or Board prior to participating in any activity or meeting consisting of three (3) or more participants.
This only needs to be completed ONCE per individual, not for each activity in which you participate.
